Personal Information

  • Valid photo ID (Driver’s license or passport)
  • Social Security numbers (including spouse & dependents)
  • Last year’s tax return (if you’re a new client)

Income Documents

  • W-2 forms from all employers
  • 1099 forms (freelance, contract work, interest, dividends, unemployment, etc.)
  • SSA-1099 (Social Security income)
  • K-1s from partnerships, S corps, or trusts
  • Records of rental income, if applicable
  • Alimony received (if applicable)
  • Any Other Income Documents

Deduction & Credit Documents

  • 1098 forms (mortgage interest, student loan interest)
  • Property tax receipts
  • Medical and dental expense records
  • Charitable contribution receipts
  • Childcare expenses (including provider’s EIN or SSN)
  • Education expenses (Form 1098-T and receipts)
  • IRA or HSA contributions
  • Proof of health insurance (Form 1095-A/B/C)

Other

  • Bank account info for direct deposit
  • Any IRS or state tax letters received
  • Estimated tax payment receipts
